- Notice from employer
- COBRA enrollment forms

Contact the company that serviced the previous health-care coverage that was provided by the employer. The insurance company is required to contact the newly qualified person of their eligibility for COBRA coverage within 14 days of being notified by the employer that the group-health-insurance coverage has ended.

Request the forms. Usually the insurance provider will automatically provide the forms that are needed to select COBRA coverage.
